How does one apply for a transfer to another store?

First, you need to find a store that will take you.  Second, make sure your original store will let you leave.  Unfortunately, you are at the mercy of your store manager, as it is up to him/her if they will release you.  If they will not release you, you are screwed because at least in Cincinnati/Dayton, the union will not help you.

If all stores in Cincy/Dayton are like mine, the stores don't have enough staff to let anyone get released to another store. Not when you have a line of department heads wanting to step down but aren't allowed to when there's no one to fill the spots.

You can't transfer unless you have stayed 200 days at one store. Once that time is up, you can transfer to a store only 100 miles from the original store.
