How long have you've been going through this? Your paperwork should've   in your seventh month. At least that's what my store's union rep told me. My advice is to keep pestering them til they give you your papers, dont leave the office until they do. You should leave there with THREE forms to fill. One for only you and the other two by you and the doctor.

Also if you're insured with Kaiser it's going to take about 2 weeks for them to fax the forms back to your store. Yeah, they're not going to fill out the paperwork in front of you. They esign with their own special forms from the hall of medical records which takes 6 to 10 days to process and fax.

Oh! And make sure your HR gets it. Sometimes papers that are faxed in tossed in the trash by accident. If that does happen, your doctor's office should have a set of the papers on file too. Good luck and congrats.
