I usually don't mind being a team player, my coworker says she doesn't lie and her speach says that but her actions also dictate other wise. More than once she's asked to change or alter shifts. I usually don't mind but she's always asking last minute. More than once she's said "I have a major medical appointment I need to do..." Shows me the email for the appointment and everything. Then I find out for more than one appointment she's played hookie and skipped. Dear, if you do this one more time I will start saying "No" to shift swaps.

I wouldn't give her anymore chances.  One chance would have been enough for me to start saying no.  She's aware of these appointments well ahead of time.  She can put in a request to be off on that day.  Even if you don't mind, it's the principle of the thing.  It lets her know you're not a pushover.
