I can only speculate, but most likely this person was the CSM or Asst.CSM with a CSM and other Asst.CSM's without a clue! Their FE Coordinator .....most likley wasn't wasn't paying attention to Weekly/Period Reports.
I currently don't know of anything in a KMA or GO what actually tracks LOSS to actually investigate issues like this!
