return to work note?

ive been on a LOA since january, and i have to turn in a return to work note by tuesday. i called management and they said i have to get the doctor to write one and there was no paperwork from them involved, but when i called my doctor, they said i needed some sort of paperwork from kroger. i am confused. does anyone have any experience with this?

Call the Div Office and speak to HR, if you received information from Metlife look in that packet for a"return to work" form.
