Serious Incident Report.

Call your regional union representative and ask.  Don't take anyones' word for it.

If you signed it, it is harder for the union to fight it.

At my store (about 3 or 4 years ago) I got one and it was marked as a "Significant Incident Reminder".  (I think!)  It seems like in the fine print it explained it could be something good or bad, and the paper would be filed away in my employee folder, in the office filing cabinet.  In my case, I felt like it was EXTREMELY INSIGNIFICANT!!

So am I forgetting the exact terminology the initials S I R stands for?    Or do some stores actually do this differently?  With Kroger being involved, it wouldn't surprise me!

You are right.  I stand corrected. I have never had one.

I can't remember which is worse: SIR or CAR.  I tell everyone to not sign anything.  That is an admission of the write up being correct and the union is less likely to fight it.

One of our new at the time co managers wrote me up for an incident that never happened. I was coerced into signing it. The Union successfully fought it reduced it to a Note to File and it disappears after I think one (?)  year. She ended up with a LOT of grief from our store manager (who was instrumental in getting this knocked out) and the regional office who called her into there for a meeting after way too many filed complaints with the union. It CAN be done but you MUST inform your Union rep right away!!

Yes, it's true. It isn't a write-up, it is a warning/significant reminder (or something of that nature).
